Download and complete the following forms. Once completed upload to Camper Dashboard in appropriate spot.

• 2024 Campers who submitted a PE form with a physical exam date of 2024 can be rolled over to this summer. Parents, email the office to do so. IF your child has any updated medications not listed on their PE from last summer, a new form is required.
• ALL NEW campers for this summer are required to have a PE form on file with us.
• ALL campers must have the TCRG form signed by their TCRG. This form should be filled out and uploaded within two weeks of registration.

1. Log into Parent account (you can only log in if you have already registered.  If you have not registered yet, please click here to register) 
2. Save documents as a PDF or JPEG
3. Click upload button
