• Download and complete the following forms. Once completed upload to Camper Dashboard in appropriate spot.

• 2025 Campers who submitted a PE form with a physical exam date of 2025 can be rolled over to this summer.

  • Parents, email the office to do so.

  • IF your child has any updated medications not listed on their PE from last summer, a new form is required.

• ALL NEW campers are required to have a CRC PE form on file with us.

• ALL campers must have the TCRG form signed by their TCRG/ ADCRG. This form must be filled out and uploaded within two weeks of registration.

  • If your child levels up before camp, a NEW SIGNED TCRG form must be emailed to the office in order to update your child’s level on their registration.

1. Log into Parent account (you can only log in if you have already registered.  If you have not registered yet, please click here to register) 
2. Save documents as a PDF or JPEG
3. Click upload button
